The American Geriatrics Society and the AGS Foundation for Health in Aging occasionally partners with other non-profit organizations to help enhance and further our mission. Our collaborative partnerships are with organizations of similar focus, which is to improve the care of older adults through research, public education and advocacy.
National Transitions of Care Coalition
As a coalition member, AGS/FHA works with thought leaders, patient advocates, and health care providers from various care settings to improve the quality of care coordination and communication when patients are transferred from one level of care to another.
Alzheimer's Foundation of America's 6th Annual National Memory Screening Day
National Memory Screening Day (November 18, 2008) is one of the highlights of AFA's ongoing national effort to promote early detection of Alzheimer's disease and related illnesses, and to encourage appropriate intervention, including medical treatments, social services and other resources. AFA believes that memory screenings are a significant first step toward early diagnosis.
AFA carries out this event in collaboration with organizations and healthcare professionals across the U.S.—bringing them together for care. Participating sites offer free confidential memory screenings, as well as follow-up resources and educational materials to those concerned about memory loss.
Help Seniors Live Better, Longer: Prevent Brain Injury An initiative of the Centers for Disease Control and Prevention [CDC]
The FHA joins the CDC in their efforts to raise awareness about ways to prevent, recognize and respond to fall related traumatic brain injury in older adults.
New York Junior League
This project with the New York Junior League disseminates health information to older adults in the community through presentations by trained volunteers.
Life In Motion Coalition
Spearheaded by WE MOVE, this coalition of more than 40 professional and patient advocacy groups to launched Life in Motion - a campaign to raise awareness about the more than 30 neurological conditions that may be classified as movement disorders, and encourage earlier diagnosis and treatment.
N4A's Making the Link
A project of the National Association of Area Agencies on Aging (n4a), Making the Link brings together two distinct but complementary systems - the aging network and health care providers - to benefit family caregivers across the country.
NCOA Falls Free Coalition
As a member of the NCOA's Falls Free Coalition, the FHA participates in consensus meetings to help shape materials & guidelines for falls prevention.
